Resources and Documents
All incoming 6th graders and any athlete who does not have a physical on file from last year, must have a physical completed prior to tryout dates this year.
Any returning athletes who had a physical on file from last year needs to complete this form and other the documents before competing.

- In order to participate, student athletes must be passing six classes and have a completed physical on file with the school. Physicals must be signed by a physician. Physicals examinations must be completed between April 1 and the first tryout or practice in order for a student to participate.
- Eighth grade- Any regularly enrolled eighth grade student, who has not reached age 16 by September 1, is eligible to participate for the entire school year.
- Seventh grade – Any regularly enrolled seventh grade student, who has not reached age 15 by September 1, is eligible to participate for the entire school year.
- Sixth grade – Any regularly enrolled sixth grade student, who has not reached age 14 by September 1, is eligible to participate for the entire school year.
